New "Custom Lists" filter on Physician Group Explore

WHAT'S NEW
The Physician Group Explore tab now includes a "Custom Lists" filter, so you can narrow the grid to only the physician groups already saved in your lists.
WHY IT MATTERS
You get the same custom-list filtering you already rely on elsewhere in the platform, letting you jump straight to the groups that matter to your territory or strategy instead of scrolling through the full grid.
Facility Explore metric renamed for clarity

- HHA: "HHA Discharge Non-Adherence %" is now "HHA Patients Non-Adherence %"
- SNF: "SNF Discharge Non-Adherence %" is now "SNF Patients Non-Adherence %"
- HOS: "Hospice Discharge Non-Adherence %" is now "Hospice Patients Non-Adherence %"
WHY IT MATTERS
The new name removes the common confusion between the patient-based metric on the Explore page and the episode-based adherence metric on the Analyze page — so you can trust you're comparing the right numbers when making referral and network decisions.
