Release Summary
- This update includes changes to Portal functionality and new features.

The following features and enhancements apply to both the Home Health and Hospice Portals.
Physician Detail Page - Billing NPI Table
A new table has been added to physician detail pages for both portals.
This table provides a breakout of the physician's claims attributed to the listed billing NPIs by percentage. The Physician Affiliation table has been moved from its position near the top to be set alongside this new table.
The enhancements in this section only apply to the Hospice Portal.
Facility Detail Page - New Metric Name
The metric "Hospice Use %" has been re-named to "% Mortalities Admitted to Hospice," in the Key Stats Table.
Hospice Detail Page - Quality Metrics - New Metrics
The State and County Comparison metrics have been added to the table for % Hospitalized and Average Length of Stay.
Physician Detail Page - Table Name Change - Hospice Utilization
The table; "Hospice Utilization and Preventable Hospital Visits," has been shortened to Hospice Utilization. This was done in conjunction with the new table listed immediately below. The new table contains the end of life hospitalization metrics that were in the original combined table. (See screenshot below)
Physician Detail Page - Average Hospital Visits
Average Hospital Visits is a new table that has been added to show how the length of hospice stays impact hospital utilization in the last 30 days of life. This table shows the Average Hospital Visits broken out by encounter type (Inpatient, Inpatient ER, Outpatient ER, or Overnight Observation) for patients receiving early hospice (admitted more than 30 days prior to death), late hospice (admitted 30 days or less prior to death), or no hospice.
All content in this section pertains only to the Home Health Portal.
New Physician Engagement Report - Physicians Affiliated with My Agency
We have added an additional report that can be created from your "My Dashboard" page. You will set up this report to identify physicians who have a closer affiliation with a chosen agency or set of agencies. This will allow you to identify physicians with whom the selected agency or agencies share a significant patient population.
My Dashboard Reports - Range Filters
The range filters can now be set to limit metrics from both the top of the scale as well as the bottom of the scale. This allows you to limit the items listed in the report to those that match specific criteria.
Report - Total Cost of Care - Enhanced Breakout Data
The Total Cost of Care Report has three sections: A section to Add/Edit organizations to the table, a table of your selected organizations, and a chart that allows you to compare those organizations by the total cost of care. This change will appear in the table and in the chart. In the table, these are reflected in average counts. In the chart, the new categories include totals in US Dollars.
In the previous iteration of this page, there was a category called "Other Payment Amount, (See next screenshot). In this release, the contents from that previous category will be broken out into additional categories.
Those categories are:
- OP (Non-ER) Claims - These metrics are related to outpatient (OUTP) claims that are not OBS/ER.
- HOS Claims - Costs from Hospice Claims
- Prof Claims - Costs from Professional Claims
- DME Claims - Durable Medical Equipment expenses.
Report - Total Cost of Care - Cost Data Export
We have added the capability to export the cost data from the chart.
Report - Total Cost of Care - Removal of Negative Claims from totals
All claims with a negative payment amount were removed from the data. This will produce more accurate Total Cost of Care calculations. This may result in some facilities reflecting a lower or higher readmission or hospitalization rate, if episodes of care considered for discharge and/or readmission were both removed from the calculation. In the end, however, this will provide a precise picture of the costs.
Home Health Detail Page - Home Health Agency's Summary Table - Calculation Change
Our data scientists tweaked this metric to bring it into alignment with the total row on the Readmission Rate by Major Diagnostic Category table.
