The SNF Destinations table on the Facility Detail page lists all of the skilled nursing facilities that admitted patients within 30 days of discharge from the facility on the main page. The table has three tabs that break out the metrics based on the care setting under which the facility submitted claims. Inpatient/Outpatient combines the Inpatient and Outpatient claim metrics found in the next two tabs.
Understanding the Data
|SNF Name/NPI/ZIP||This name is a link to the detail page for the listed SNF. If needed, you can add an alias to this name to make it searchable by another name. The NPI is the National Provider Identifier number of the SNF receiving patients from the facility, maintained by CMS. The ZIP code is for the primary facility using the NPI.|
|Total||This is the total number of distinct patients discharged during the one year reporting period who were admitted to the SNF within 30 days.|
|Level of Affiliation (Only on Inpatient/Outpatient Tab)||This metric is the percentage of patients for the SNF in the row measured against the total number of patients discharged by the facility and admitted to all SNFs in the table. Simply it is the number in the row divided by the number at the bottom times 100%. This shows the relative population of patients received by the SNF in each row.|
|Quarterly counts - Year-Q#||This is the count of distinct patients discharged from the facility on this page who were admitted to the SNF in each row during the quarter identified in the column header.|
|Average Medicare Days per Episode (Not in Outpatient)||The average SNF episode duration for patients discharged from the facility and then admitted by the named SNF and later discharged from their SNF stay within the one year reporting period. This metric does not account for portions of a beneficiary's stay billed under non-Medicare payment types.|
|% Hospitalized: +30 days.||SNF’s two year overall rate of hospitalization (where this agency’s patient was admitted to a short term acute stay during an SNF services episode (includes transfers) OR within 30 days of discharge from the SNF)|
