The Excel Health Solutions contain numerous metrics that reveal the flow of patients between physician or facility sources and hospice or home health agency destinations. Each entry in the main tables has a green and white triangle button that opens a table of destinations for physicians or facilities, and sources for hospices and home health agencies. In some cases, the numbers don't seem to add up, and this article addresses a specific example.
Notice the Fulton County Health Center in the image below. The column of patients entering hospice within 30 days indicates a count of 22 patients. In the destinations table, once opened, the counts of patients who received hospice care is the column in the rectangle. How did 22 patients become 28 + 23 + 19 + 19 + ....
The answer is in the title of the destination table: the patient counts include Inpatient, Outpatient AND SNF patients.
If we take the NPI for Fulton County and add it to the search field using the "All Facilities" tab, we find that there are three entries for the Fulton County Health Center. The one NPI covers Inpatient, Outpatient and SNF claims, so we can see that there are 150 patients from the single facility source.
This happens because we split up the facilities into different tabs so that the facility types can be viewed together. At the same time, the destination tables always contain all patients from a specific NPI.
