Second, you can dig into facility metrics and build an account plan that gives you the best chance for strengthening your relationship. Finally, the data can support conversations that position you as a trusted adviser that understands their challenges and has a plan for how working with your home health agency can help them improve.
Finding the right facilities often requires delving deeper into the patient destinations table or viewing the detail page for those facilities. The following list summarizes a few approaches to consider. These are examined in more detail in the articles for specific tables or pages.
- Home Health and SNF Destinations - Identify which home health agencies or SNFs treated patients after discharge from each facility. Learn the facility's approach to partnering with home health or SNFs and where you rank with them.
- Post-Acute Destinations/Discharge Metrics - Discover the rate at which the facility discharges to a given setting and patient adherence with discharge coding. Identify challenges in readmission and mortality for each discharge setting.
- Discharge Metrics by Diagnostic Category - Isolate diagnostic categories that are a challenge for the hospital or SNF, so you can present your approach to solving a meaningful challenge within a specific department.

Each facility; hospital or SNF, has a detail page with metrics that are primarily focused on the facility as a source of home health patients. Sections are outlined in the table below.

|Contact Information||This section contains the detailed identification information for the facility on the page. Some of this information can be edited. This is where you would add an alias for the facility.|
|Key Staff||In order to keep the most important contacts from this facility available to your team, your team members are able to add people to this table. Entries can include anyone including all physicians listed in the Solution.|
|Key Stats||This table includes summary metrics for the facility on the page with comparative state and county averages.|
|Post Acute Destinations||This table contains insights into patient discharges to post acute care and which post acute care patients actually received. This table is identical in the Hospice and Home Health Solutions.|
|Patients Discharged with Home Health Instructions (This table only applies to Inpatient.)||
This table provides additional details about patients who were coded at discharge for post-acute care at a home health agency. This table clearly shows the advantage to a hospital when patients receive home health care.
This table should not be construed to suggest that any actual instructions were provided to any patient. The metrics in this table only identify the status coding at discharge.
|Home Health Destinations||This table includes a list of all home health agencies that admitted patients from the selected facility within 30 days of discharge. This table can be viewed with Inpatient and Outpatient claims separated or counted together. The metrics provide a quick view of relative patient counts and performance metrics for evaluating and/or comparing home health agencies that admitted patients from the facility on the page.|
|SNF Destinations||This table is similar to the Home Health Destinations table but includes the counts and performance metrics for patients from the facility on this page who were admitted to the skilled nursing facilities listed in the table.|
|Patient Diagnostic Mix by Distinct patients||This table provides a breakdown of several metrics against the 20 Trella Health Diagnostic Groupings. For some metrics we have included comparative county and state averages.|
|Patient Classification||This section contains five tables with corresponding pie charts that reveal significant patient demographics for the facility listed on the page. Location demographics are relative to patient residence, not facility address.|
