The Physicians Page is your entry point into the physician data in the Trella Health Home Health Solution. Before calling on a physician, search the Main Table for the physician and review their details to improve your interaction with them. Alternatively, you can use Physician Targeting Strategies – Home Health (HHA) to identify physicians who are attractive targets for business development.

Home Health Destinations
The Main table has two layers of data. The first layer includes a list of all physicians in your selected region. The second layer provides additional information for the physician in each row. To the left of every physician name is a green square with a white triangle. Clicking on this button will open a table of Home Health destinations for the patients of the selected physician. The listed Home Health Agencies admitted patients treated by the physician in the row.
Physicians with the specialties; Radiology, Pathology, and Anesthesiology will not have any data in their Home Health Destinations table. Although these physicians submit a lot of claims, they have little patient contact and are therefore not good candidates for generating referrals.

Understanding the Data
The Physicians Main table lists all physicians in your selected region. Each row of the table contains a physician with important identification information and metrics that will help you to understand the demographics of that physician’s patient population and also to develop some insights into the informal referral relationships between the physician and the patients who received Home Health care.
|Physician Name/NPI||Physician Name and NPI, as listed in CMS Provider Data (NPI) file.|
|County/ZIP||Primary County/ZIP code of physician, as listed in the NPI file. In this table, counts are placed in the ZIP code of the Physician's practice, not the residence of the patients.|
Physician's self-reported primary specialty. You can add a Specialty Alias to this field to correct a physician's specialty and make it searchable by the specialty of your choice.
|% of Total Claims (Billing NPI)||
This column is only populated if a physician practice is selected in the Billing NPI filter. This is the percent of claims submitted by the selected billing NPI that originated from the physician listed in the row. for more information, see Billing NPI Filter.
|Patient Counts||Physician Medicare Patients||This date is a count of distinct Medicare patients treated by this physician from claims submitted during the one year reporting period (last four quarters). A single distinct patient may have multiple claims for a physician but will only be counted once.|
|Physician Home Health Patients||This is a cross section count* of the physician's Home Health patients from Followed, 3 Months prior, and Last Claimed counts during the one year reporting period (last four quarters). Patients counted in this column were admitted to Home Health care during the reporting period.|
|Followed Patients||This count comes from Home Health End of Episode claims for Home care episodes that started during the one-year reporting period (last four quarters) where the physician is listed as the following physician.|
|Patients 3 Months Prior||A physician receives a count in this column if they had submitted a claim for the patient during the three-month reference period prior to admission to Home Health Care, based on the date of the start of care. This period is limited to three months to identify physicians who have a closer possible connection between the care they provided and the patient’s entry to Home Health care.|
|Last Claimed Patients||A physician receives a count in this column if they submitted the most recent claim for the patient prior to the patient's admission to Home Health care. Although not conclusive, this represents the highest likelihood of a connection between a physician’s prior care and admission to Home Health care.|
|Averages||Days Per Episode||The average Home Health episode duration for patients who were discharged from a Home Health stay during the one year reporting period (last four quarters) who were also treated by the named physician within 3 months of their Home Health admission.|
|Episodes Per Patient||The average number of concurrent Home Health episodes per patient for patients discharged from a Home Health stay during the one year reporting period who were also treated by the named physician within 3 months of their Home Health admission.|
|Average Visits Per Episode||Nurses||Average number of skilled nursing visits per Home Health episode for patients who were treated by the named physician within 3 months of their Home Health admission|
|Therapists||Average number of therapy (PT, OT, SLP) visits per Home Health episode for patients who were treated by the named physician within 3 months of their Home Health admission|
* This count is a cross section because there might be claims for a physician/patient pair that are counted in multiple columns - Last Claimed, 3 Months Prior, and Followed - which would only be counted once in the Physician Home Health Patients column.
